There’s a lot involved in managing a chronic condition. livingwellCARES offers free, confidential one-on-one chronic care management to employees and family members. Even better—you may save on what you pay for condition-related medication and supplies.

Specially trained nurses partner with you and your doctors. You speak with them often, either by phone, virtually, or in person. They help you schedule appointments, manage medications, and support the changes you may need to make to manage your health condition successfully. Best of all, they offer the inspiration you and your family need to achieve your wellness goals.

Unless you opt out during Open Enrollment, you may receive an invitation to participate if your recent medical claims reflect a diabetes or high blood pressure diagnosis. You can also enroll on your own. Either way, participating is confidential and voluntary. Your personal health information is protected by HIPAA; neither management nor Human Resources have access to that information.
