YNHHS Care Management Program
The YNHHS care management program is staffed by YNHHS clinicians and is provided at no cost, It includes several components:
- A three-month voluntary program for patients with multiple medical conditions and high clinical risk, which includes weekly 20- to 30-minute calls with a YNHHS care management program nurse to work on goals that are meaningful to you, like emotional support, nutrition, exercise, medication-related, social and disease-specific needs. Members who are eligible will be contacted by the care management team.
- Increased support for transitions of care after hospitalization, and
- Support for preventive health care.
If you meet eligibility criteria, a YNHHS care management program nurse will reach out to you directly to work with you to manage your condition and help you navigate your health care needs.
What is a Chronic Condition?
A chronic condition is a long-term illness, which requires a person to adjust to the demands of the illness and the therapy used to treat the condition. Examples of chronic conditions include, but are not limited to: heart disease, type 2 diabetes, obesity, arthritis, depression and joint pain.
This benefit is available to employees enrolled in the YNHHS Medical Plan.
