Predetermination of Benefits
Whether you are seeking services in- or out-of-network, if your procedure will cost more than $300, ask your provider to submit a pre-treatment estimate to Delta Dental. That way, you’ll know how much the plan will cover and how much you’ll need to pay. Call Delta Dental at 800-452-9310 with any questions.
Out-of-Network Dental Care
If you receive care from a dental provider outside the Delta Dental network:
- You may pay more for services because non-participating dentists have not negotiated fee discounts with Delta Dental.
- You may need to pay the dentist yourself and then submit a claim to be reimbursed by Delta Dental.
- If you need to submit a dental claim yourself, ask your dentist for a standard American Dental Association claim form.
